The form is a PDF document that can be filled out, saved, and then printed.

Note that you MUST sign the form if you are paying by credit card, otherwise the form will be returned to you.

Mail your completed form along with either a check or your credit card information directly to the NMRA address on the form.

Note that all Early Registration periods have now ended.
